BackgroundCrestor (CRESTOR) is a widely used statin for heart failure and is also approved for use in combination with other statins. It is prescribed primarily for patients with moderate-to-severe heart failure who have not achieved the goal of achieving an adequate reduction in fluid and electrolyte levels. In addition to this use in combination with other statins, CRESTOR can also be prescribed for patients with advanced-stage HF, as well as patients at risk of cardiovascular events. CRESTOR has been shown to be effective in improving survival in patients with decompensated heart failure. CRESTOR has been extensively studied in patients with severe HF who have not achieved their target reduction in fluid and electrolyte levels and/or preserved ejection fraction (eGFR). The use of CRESTOR in these patients is generally considered safe and has been extensively researched. However, there is currently limited research evaluating the effects of CRESTOR on the overall survival (OS) and survival in patients with decompensated heart failure with preserved ejection fraction (HFpEF) at the time of presentation. This study was conducted to evaluate the effect of CRESTOR on the OS of patients with decompensated HFpEF with preserved ejection fraction (HFpEF-pEF), as well as to assess whether CRESTOR is associated with an increase in mortality and overall survival. A prospective, multicentre, randomized, double blind, placebo controlled trial was performed in this study. Data was collected on all patients presenting to our clinic at the beginning of the trial for a period of 12 weeks. We assessed the treatment-emergent adverse events (AEEs) using the International Classification of Adverse Events (ICAE) and overall survival (OS) using the Kaplan-Meier method. Data was analyzed using the intention to treat model and compared using the log-rank test. Patients were included in the study if they met the following criteria: 1) they had a baseline clinical diagnosis of heart failure (HFpEF), including decompensated HF (HFpEF-pEF ≥ 40) and/or HF failure (HFpEF ≥ 20) plus a baseline creatinine ≥ 1.9 mg/dL (defined as a Creatinine <1.9 mg/dL for a defined range of 1.0 to ≥2.0 mg/dL); 2) a baseline ejection fraction (EF) <20% and at least 2% at baseline, with a baseline creatinine >1.2 mg/dL (defined as a creatinine >1.2 mg/dL with a defined range of 0.4 to 2.0 mg/dL); 3) a baseline body mass index (>30 kg/m2 with a defined range of 28 to 29 kg/m2); and 4) a baseline hemoglobin >12 g/dL (defined as a hemoglobin >12 g/dL). CRESTOR was started at a dose of 10 mg/day with subsequent doses titrated up to 20 mg/day. The primary outcome was a reduction in the change from baseline in the number of days on treatment with CRESTOR. There were no significant differences between the groups in terms of change from baseline in the number of days on treatment with CRESTOR. There were significantly fewer patients in the CRESTOR group who had experienced AEs/AEs in the course of the trial (median 8; IQR 2 to 12). The primary outcome measure was the change in the number of days on CRESTOR (defined as the number of days on treatment with CRESTOR divided by the number of days on treatment with placebo). The results from this trial showed that CRESTOR reduced the number of days on treatment with CRESTOR from 4 to 12, but this was not significantly different from placebo in terms of change from baseline in the number of days on treatment with CRESTOR (3). AEs/AEs from the CRESTOR group were more common in patients with HFpEF-pEF > 40, and in patients with HFpEF-pEF > 20. AEs in the course of the trial were less common in patients with HFpEF-pEF > 40, and in patients with HFpEF-pEF > 20. The results from this trial suggest that CRESTOR is safe in patients with HFpEF-pEF > 20 and is safe in patients with HFpEF-pEF > 40. This is an important study because the number of patients with HFpEF-pEF > 40 with preserved ejection fraction was small.
